Professional practice assessment task 1:
As part of professional practice assessment task 1, you should perform nursing admission procedures for a person in your workplace complying with your workplace policies and procedures under supervision- This assessment may be undertaken as part of the unit of competency HLTAAP002 Confirm physical health status- Use the following admission template and report section for related documentation-Appendix 1 provides you with further information on what information you could collect from the person- Use this appendix as a guide-
The Host Facilitator or EQUALS' Clinical Facilitator will observe your skills in performing the admission procedures and complete the observation checklist and assessment outcome sections -provided in the logbook- based on the skills you demonstrated, and the documentation completed-
Professional practice assessment task 2:
Develop a nursing care plan for a person in your workplace, based on the holistic health assessment -physical health assessment and health history collection-- You may prefer to develop a nursing care plan for the person you admitted as part of task 1 or any other person, as appropriate-
If a nursing care plan is already developed for this person, you must not refer to this document- Perform required assessments and gather sufficient information and develop a plan of care for the person- Then, compare your plan of care with the nursing care plan already developed for the person- Reflect on and raise with the registered nurse or interdisciplinary health care team any conflicts between the nursing care plan you developed and an already prescribed plan of care for the person-
If a nursing care plan is not yet prescribed for this person, wait for the nursing care plan to be developed and then undertake a comparison- Alternatively, you could discuss the plan of care you developed with the registered nurse and/or interdisciplinary health care team and discuss any conflicts between the nursing care plan you developed, and the actual care required for the person-
Use the nursing care plan report template provided below for appropriate documentation of the plan of care and reflective report-
Professional practice assessment task 3:
As part of professional practice assessment task 3, you should perform nursing discharge procedures for a person in your workplace complying with your workplace policies and procedures under supervision-
Gather sufficient information and perform holistic health assessments using appropriate equipment and techniques to facilitate the discharge process-
Use the following discharge template and report section for related documentation-
The Host Facilitator or EQUALS' Clinical Facilitator will observe your skills in performing the discharge procedures and complete the observation checklist and assessment outcome sections -provided in the logbook- based on the skills you demonstrated and the documentation completed-
Professional practice assessment task 4:
You must implement, monitor and evaluate two -2- different nursing care plans for two -2- clients under the supervision of a registered nurse in the workplace- You must work with two -2- clients for a significant period of time -half a day, a shift etcetera- and undertake relevant activities outlined in the nursing care plan within your own scope of practice-
You must undertake the following activities, where appropriate, when implementing nursing care plans:• Appropriately refer to the nursing care plan• Prioritise work activities using a time planner• Maintain a balanced approach to your workload in the workplace• Ensure sufficient resources are available to provide required nursing care• Seek support if any activity outlined in the plan of care is beyond your scope of practice• Appropriately prepare clients for care procedures• Explain the procedures• Seek consent• Maintain the person's privacy and dignity• Assist clients to address their activities of daily living• Use identified aids to assist activities of daily living, where appropriate-• Contribute to their own independence and mobility-
You must undertake the following activities, where appropriate, when monitoring nursing care plans:• Maintain ongoing assessment of the person• Use various techniques such as observation and use of monitoring equipment and devices• Prioritise work activities to address the person's urgent needs• Record and report concerns in the person's condition or behaviour to the registered nurse• Report situations of risk according to organisation policy and procedures-
You must undertake the following activities, where appropriate, when evaluating nursing care plans:• Identify and evaluate own contributions to the person's care in collaboration with the interdisciplinary team-• Evaluate the person's progress towards planned outcomes as documented in the care plan-• Where appropriate, make required changes to the nursing care plan in line with best practice in nursing-• Record and report all aspects of the evaluation of nursing care provided-
You must complete the following templates provided for Activities 1 and 2 -for care plan 1 and 2- for each client, as applicable-
Professional practice assessment task 5:
Refer to your workplace policies and procedures and engage in discussion with interdisciplinary team members to understand the recording and reporting requirements for comprehensive clinical handovers and case meetings-Summarise your findings in the space provided below-The Host Facilitator or EQUALS' Clinical Facilitator will complete the observation checklist and assessment outcome sections -provided in the logbook--
